Privacy Policy
This policy explains what Christensen Prosthodontics collects through this website, why we collect it, and what we do with it. It was last updated on September 23, 2026.
Who this policy covers
This website is operated by Christensen Prosthodontics, a prosthodontic practice at 1375 East 800 North, Suite 204, Orem, UT 84097. In this policy, "we" and "our" mean the practice, and "you" means anyone who visits this website or sends us information through it.
This policy covers the website only. Health information that becomes part of your dental record is handled under federal health privacy law and under our Notice of Privacy Practices, which we provide at your first visit and can send to you on request. If the two documents ever disagree about your health information, the Notice of Privacy Practices is the one that governs.
What we collect
Information you send us
Our consultation request form asks for your first name, last name, phone number, email address, and the type of care you are interested in. Your name, phone number, and email address are required so that we can reply. Everything else is optional.
Our referral form, which is meant for dentists referring a patient to us, asks for the referring dentist's name, practice, phone number, and email address, and for the patient's name, phone number, email address, reason for referral, and any notes the dentist chooses to add.
If you email us or call the number on this site, we receive whatever you choose to tell us.
Information collected automatically
Our web server keeps standard logs of requests made to the site. These logs can include your IP address, the pages you requested, the time of the request, and basic information about your browser. They are used to keep the site running and secure.
Cookies and analytics
This website uses Google Analytics to understand how the site is used, for example which pages people read and whether they find what they came for. Google Analytics sets cookies on your device, named _ga and similar, which give your browser a random identifier. We use this only in aggregate. We do not use it to identify you, and we do not combine it with anything you send us through a form.
We do not use advertising cookies, we do not run retargeting pixels, and we do not build a profile of you across other websites.
If you prefer not to be measured, you can block cookies in your browser settings or install the Google Analytics opt-out add-on. Blocking them does not affect how the website works for you.
Some files used to display the site, such as fonts and animation code, are delivered by other companies' networks, specifically Adobe Fonts, Cloudflare, and unpkg. When your browser requests those files, those companies receive your IP address and basic browser information as part of the request. We do not send them your name or your form details.
How we use your information
We use what you send us to do the following:
- Reply to you and arrange a consultation or appointment.
- Answer a question you asked, or follow up on a referral from your dentist.
- Keep an internal record that your request came in and how we handled it.
- Keep the website secure and working properly.
- Meet our legal and professional obligations.
We do not sell your personal information. We do not rent or trade it. We do not use it for advertising, and we do not send marketing email to people who contacted us only to ask a question.
Please do not send detailed health information through this website
The forms on this website are for getting in touch, not for describing your health history. Email and web forms are convenient, but they are not a secure way to send sensitive medical detail, and we cannot control how messages travel across the internet or who can see your device or inbox.
Send us enough to start the conversation. If we need clinical detail, we will collect it at your visit, or by another method that protects it properly.
Who we share it with
We share your information only in these situations:
- Companies that help us run the practice and this website. This includes our website hosting provider, our email provider, and Google, which provides the analytics described above. They may handle your information in the course of providing that service, and they are not permitted to use it for their own purposes.
- Your dentist, or a specialist involved in your care, where that is the point of your message, such as a referral.
- When the law requires it, for example in response to a lawful request from a public authority.
We do not share your information with advertisers or data brokers.
How long we keep it
Messages sent through the website are stored in the website's database and are also sent to the practice by email. We keep them for as long as we need them to respond to you and to keep a reasonable record of the request, and we delete them when they are no longer needed. If your enquiry leads to treatment, information that becomes part of your dental record is kept for as long as the law and our professional obligations require.
How we protect it
This website is served over an encrypted connection, so information you type into a form is encrypted while it travels to our website. Access to the website's administration is limited to people who need it.
We want to be straightforward about the limits. Notification emails that tell us a form has been submitted travel by ordinary email, which is not encrypted end to end. No website or email system can be guaranteed to be completely secure. That is the reason for the request above: please keep detailed health information out of the form.
Your choices
You can ask us to tell you what information we hold about you, to correct it if it is wrong, or to delete a message you sent us through the website. Requests about your dental record are handled under our Notice of Privacy Practices, which explains your rights over that record.
To make a request, contact us using the details at the bottom of this page. We may need to verify who you are before we act, particularly where health information is involved.
You can set your browser to block or delete cookies, which will stop the analytics described above. The website itself will keep working normally.
Children
This website is meant for adults. It is not directed at children, and we do not knowingly collect information from a child through it. We do treat children as patients, and a parent or guardian provides that information to us directly rather than through this site. If you believe a child has sent us information through the website, contact us and we will delete it.
Links to other websites
Some pages link to other organizations, such as the American Dental Association or the American College of Prosthodontists, and to Google Maps for directions. Those sites have their own privacy policies, and this policy does not apply once you leave our website.
Changes to this policy
If we change how this website handles information, we will update this page and change the date at the top. If a change is significant, we will make that clear rather than quietly editing the text.
